Consider this before you get started…

• • You must complete all Donor Ready steps before you can reserve a donor. Learn more about these steps here.
  • We strongly encourage you to request three of your preferred donor matches at the time of submission, as sperm availability is subject to change due to supply and demand.
  • Please note that you must have a confirmed reservation by Day 1 of your period; otherwise, you will be required to wait until your next cycle.
  • Please note that certain medical conditions or carrier status may determine the suitability of some donors over others. Please discuss this matter with our fertility specialist during your appointment. 
  • Once your donor request is submitted, our ADDAM team will review your request and respond with an outcome within 5 business days.
  • If successful, the ADDAM team will confirm your reservation via email. Payment must be made in full by 3 pm the following day. No extensions will be given for payments. If no payment is received within the required timeframe, the reservation will be cancelled, and the donor will be returned to the donor pool for other recipients to use.
  • Once payment is made, a refund of the reservation fee or of unused donor sperm is not available.
  • Please note: International Victorian donors are only available to Victorian residents. Interstate travel to access Victorian donors is not permitted, as these donors have been specifically imported into Victoria to comply with Victorian Department of Health regulations.
